· Who is collecting your personal data?
· Your personal data is collected by Ciara Glaisher, Chartered Physiotherapist.
· What data is being collected?
· Personal data comprises your/the client’s name, telephone number, address, animal’s address if different. An email address may also be collected. The animal to be treated will also have name, age, breed and use collected.
· What is the legal basis for processing the data?
· The client’s data is processed lawfully in order to provide the physiotherapy services requested by the client or veterinarian.
· Will the data be shared with any third parties?
· The client’s personal data may only be confirmed, as opposed to given for the first time, to the registered veterinarian. If splinting is required as part of treatment, the client’s data would also be shared with the company that manufactures splints, as part of their order processing. No other third parties will be given any data.
· How will the information be used?
· The telephone or email data is required to be able to contact you/the client, in order to arrange appointments, potentially discuss the animal’s treatment/management, rearrange appointments if necessary. Addresses are required to visit the animal/client, and to identify the client to the Veterinarian, in order to ensure consent is gained for the correct animal. It is a legal obligation to gain veterinary consent for any physiotherapeutic treatment of an animal (Veterinary Surgeons Act, 1966).
· How long will the data be stored for?
· The data will be kept for the duration of the service requirement, plus 7 years following the patient’s discharge. This is in accordance with healthcare records, and is due to the fact that the client may re-refer for treatment, where past records can inform physiotherapeutic management. The data is stored on paper records, in a locked filing cabinet.
· What rights does the data subject have?
· You/the client has the right to be informed about the data kept and its use. You have the right to access the data kept (seeing paper records). You have the right to correct any inaccuracies in the data. You have the right to object to inclusion of any data, and to restrict the data processing. However, as the minimal personal data kept is used to provide a legal Physiotherapy service, denying consent to keep address or telephone number may result in the inability to provide the service.
· By confirming an appointment, the client is consenting to physiotherapy treatment for their animal, including consent to the Privacy Policy. The client has the right to withdraw consent at any time, to discontinue treatment. [N.B. If this occurs during a treatment session, the client will still be subject to the cost of the appointment.]
